About Gynecomastia

Gynecomastia, or man breast, developed glandular tissues, making them feel uncomfortable or self-conscious. It is a condition in which boys and men develop breast tissues due to a hormonal imbalance or high levels of estrogen. 

Gynecomastia develops slowly and there are four stages: 

  • Grade 1 Gynecomastia: The breast has only slight enlargement and a small lump can be felt in the nipple or areola. 
  • Grade 2 Gynecomastia: The breast tissues expand to the chest width and include coverage of the pectoral muscles. The enlargement may still not be prominent. 
  • Grade 3 Gynecomastia: The breasts are moderately enlarged and can be easily seen from clothes. The tissues also become firm. 
  • Grade 4 Gynecomastia: This is the worst stage of gynecomastia where the chest looks like breasts and affects the physical and psychological state. 

In addition to the above explanation, gynecomastia is also divided into true and pseudo-gynecomastia. In this type of true gynecomastia, only breast glandular tissues are present. This is different from this, in pseudo gynecomastia, only fat tissues are present in the chest and make the person’s chest look like a female’s breasts. 

Gynecomastia Diagnosis

To distinguish between real and pseudo gynecomastia and gynecomastia itself the doctor may do the following:

  • Medical History: The doctor will ask you about any symptoms you have, medical history, any medications or substances you may be taking that might have caused the condition. Some drugs that contain estrogen can cause gynecomastia are anabolic steroids, anti-androgens, or medications.
  • Physical Exam: The doctor will first start with the physical examination to check for abnormal growth of tissues, including the shape, size, and feel of the breast tissues. The test is called the pinch test. The gynecomastia pinch test is a physical examination where the examiner will grab the breast tissue between the thumb and fingers to look for any lump or mass. The physical exam may help to distinguish it, but the exam may also be inconclusive. If so, further testing will be recommended for the individual. 
  • Blood Test: A complete blood panel test may be ordered by the doctor to check on the hormone levels. 
  • Imaging Test: In order to establish the level of growth and avoid the chances of breast cancer, the doctor may ask for mammography, CT scan, MRI scan, or ultrasound. 
  • Biopsy: If the doctor may suggest a biopsy test if he/she thinks that the breast tissue growth is cancerous.
